Athletic/activity physicals

Our district's license with Privit has expired, and we have heard your feedback on the difficulty of its usage. Starting now, please use SIS to upload the necessary information for physicals:

If your athlete needs to submit a physical, please log in to SIS and follow these directions.

If your student has already played a sport last year or this past fall, you do not need to repeat this process.

Activity fee

A student activity fee of $50 will be charged to all middle school and high school students participating in athletics, cheer, dance, bowling, trap and scholar bowl. At the high school level, the fee is charged for participation in all MSHSAA sanctioned activities including orchestra, theater, speech & debate, choir and band (which includes color guard and winter guard). There will be a maximum of $100 charged to an individual student or $150 charged to an individual family, regardless of the number of students and activities. With payment of an activity fee, students will receive an activity pass, which grants entrance into high school home football and basketball games. There is also a catastrophic insurance fee of $5 for students participating in a sport or activity.
